Rabbitohs back-rower Keaon Koloamatangi and Knights centre Bradman Best will make their Origin debuts in Brad Fittler's run-on side. Cody Walker takes over at five-eighth in his first appearance since Game 3, 2020 while Eels duo Clint Gutherson and Reagan Campbell-Gillard and Knights prop Jacob Saifiti have also been recalled to the Origin arena.
Sea Eagles star Jake Trbojevic will make his 16th Origin appearance after missing the first two games due to a calf injury. Broncos prop Payne Haas was not considered due to an ankle injury.
Cowboys fullback Scott Drinkwater and Panthers enforcer Spencer Leniu have been rewarded for their good form with spots in the 19-man squad.

AJ Brimson takes over at fullback for the suspended Reece Walsh while Corey Horsburgh comes into the squad for his Origin debut, replacing Broncos forward Thomas Flegler – who is out with a foot injury. Brimson won his only Origin start back in 2020, when he deputised at fullback in the Maroons' Game 1 victory.
Reuben Cotter starts in the front row with Flegler sidelined, bringing Horsburgh on to the bench for his debut after he was part of the extended squad earlier in the series.
Xavier Coates and Murray Taulagi retain their spots on the wing despite the impressive form of Selwyn Cobbo, with the Maroons otherwise unchanged from their series-clinching win in Brisbane. Tom Dearden remains as 18th Man while Eels forward J'maine Hopgood joins the extended squad as 19th Man.
